Conducts psychosocial/mental health assessments to identify needs and formulate a patient centered treatment plan with the Veteran to meet needs and goals.

The incumbent is a clinical social worker assigned to a North Florida/South Georgia Veterans Health System in the Mental Health Clinic located in Gainesville, FL.

Provide basic information to patients about SIMEDHealth providers and services.

Observing patient and staff safety policies & procedures including HIPAA.

Retrieve voicemail messages throughout the day and return calls when necessary.

Assist in directing new patient calls to our new patient coordinator.

Shall be familiar with hospital consults & transitional care calls.

Review provider schedules on a daily basis and though out the day for availability.

Must be able to read and interpret Hospital Call Schedules.

Ability to schedule appointments, verify insurance and work an electronic fax server.
